You must have completed the online registration form. If you have not done so, please do so immediately. Your appointment may be delayed and/or rescheduled. The form will require the credit card information to confirm your credit card payment but does not process any additional payments. This is for fraud prevention. You are REQUIRED to bring the same credit card you submitted payment online along with a valid ID to your appointment. If you are not the cardholder, the cardholder MUST be present or you will be required to submit payment with another credit card. Since the onset of COVID19, payments are no longer processed in the office. Please submit payment before completing your registration form. Please DO NOT be late. We understand there are certain situations that may account for being late, therefore, please take into account the construction, weather (ex. rain) and traffic. Please plan accordingly. If you run more than 5-minutes late, your appointment is subject to reschedule, cancellation or deduction of time from your alloted appointment time. Building Access
Depending on the time of day your appointment is scheduled, you may need to enter an access code to enter our building. There is a pin pad located on the side of the door. Simply enter 2 4 6 8 and pull the door to enter. There will not be a beep or sound to acknowledge the successful entry of the pin code.
